The Gates Foundation has achieved a landmark milestone, announcing a $540.2 million donation to the University of Washington’s Institute for Health Metrics and Evaluation (IHME), establishing a new record for charitable giving in the university’s history. This substantial investment will fuel a 10-year expansion of the IHME’s Global Burden of Disease study, significantly increasing its geographic reach and data collection capabilities. The foundation’s commitment, revealed Monday, will provide the resources necessary to nearly double the study’s coverage from approximately 925 to nearly 5,000 locations worldwide, according to IHME’s stated goal of providing comprehensive data on causes of death and disease for countries across the globe. For many nations, the IHME study represents the sole source of comparable data, a crucial element for policymakers and public health practitioners seeking to understand disease trends and inform interventions. Dr. Christopher Murray, IHME’s director and professor of health metrics sciences at UW, emphasized the significance of the grant, stating, ‘This funding will allow us to provide high-quality evidence at a much more local level, enabling leaders to better understand which health decisions can have the greatest impact on people’s well-being.’

The $540.2 million donation is the largest charitable gift in UW’s history, a figure previously held by the foundation. The grant represents a significant portion of the foundation’s funding, with the rest coming from federal grants, projects with other countries, and philanthropic contributions. According to the foundation, the initial $105 million grant in 2007, which established IHME, remains the largest in UW’s history. Since then, the foundation has repeatedly set UW’s donation record, including $210 million in 2016 for the building housing the institute and $279 million in 2017. IHME’s funding from the foundation is approximately 60% of its total revenue, with the rest derived from other sources. The latest grant comes amidst growing global health funding trends, with development assistance for health declining by 21% between 2024 and 2025, primarily driven by a 67% decrease in U.S. spending. Britain, France, and Germany have also announced cuts to their philanthropic contributions, further highlighting the broader economic challenges facing the health sector. The KUOW reported in April that NIH award money promised to UW dropped 50% from fiscal 2025 to fiscal 2026, with funds in hand down 74% because of slow federal grant processing. The foundation is undergoing a transition of its own, announcing in May 2025 that it would spend $200 billion over 20 years and close on December 31, 2045. The board approved a record $9 billion budget for 2026 and a plan to cut up to 500 of its roughly 2,375 staff positions by 2030. A 10-year grant to IHME would run out approximately a decade before the foundation concludes its operations.

The grant represents a critical investment in global health research and data collection, aligning with the Gates Foundation’s broader mission to improve health outcomes worldwide. The foundation’s long-term commitment to supporting research and development in areas like disease prevention and treatment underscores its dedication to addressing global health challenges.
