The Ebola outbreak in the eastern Democratic Republic of Congo (DRC) has escalated to a second-worst on record, with the official death toll exceeding 1,500, prompting significant concern and prompting a shift in strategy for Uganda. The situation in DR Congo, however, remains precarious, with authorities acknowledging a significantly faster spread of the virus than in previous outbreaks. This shift in perspective is underscored by Uganda’s remarkable success in containing the virus, a feat largely attributed to a proactive and swift response.

Initial reports indicated a much higher number of fatalities during the outbreak, which was declared in May, and subsequent developments have further complicated the assessment. On Thursday, DR Congo confirmed that the virus was spreading considerably faster than in previous outbreaks, a development amplified by the recent release of a 12-year-old girl from a hospital – a case that sparked immediate cautious measures. The situation is further complicated by the fact that Uganda only recorded 20 cases and two deaths from Ebola in 2011, a stark contrast to the 9 reported cases and two deaths in 2000, which were primarily recorded in Uganda.

This escalating situation is a recurring theme in the DRC, with the country experiencing Ebola outbreaks since the discovery of the virus in 1976. Uganda, which has recorded 9 outbreaks since 2000, has consistently demonstrated a capability to respond effectively, though it has historically struggled to keep pace with DR Congo’s resilience. Uganda’s first case was a man from DR Congo crossing the border for treatment, prompting a rapid, coordinated response involving a specialist Ebola treatment center at Mulago hospital in Kampala.

Under the leadership of Health Minister Alan Kasujja, the first confirmed case in Uganda was a man from DR Congo, a move that immediately triggered a coordinated response. The authorities quickly activated a dedicated Ebola treatment center, utilizing leftover medical supplies and a team of trained personnel, and the facility was operational during an outbreak last year. Dr. Kasonde Mwinga, the World Health Organization’s Uganda chief, emphasized that the relatively low number of casualties wasn’t solely due to luck, but rather a deliberate investment in preparedness, a crucial element in mitigating future outbreaks.

The 17th Ebola outbreak in DR Congo since the discovery of the virus 50 years ago underscores the importance of proactive surveillance and contact tracing. Uganda, acting fast, initiated a quarantine of more than 6,000 contacts of individuals known to have Ebola during the outbreak, a critical step in halting the spread.

Uganda’s response has been particularly noteworthy, demonstrating a quicker identification and containment of the virus compared to DR Congo, which has faced challenges with surveillance and tracing. The M23 conflict in eastern DRC, with its ongoing insecurity, further complicates the situation by creating porous borders and increasing the risk of cross-border transmission. The artisanal mining and cross-border trade activities in the region also contribute to the spread, and a lack of adequate coordination has been a key factor.

Despite these challenges, Uganda has implemented a robust strategy, including sending healthcare workers to DR Congo to help their partners tackle the outbreak. Kasujja stated that the country’s success is largely due to its promptness and completeness of follow-up, adding that it is crucial for collaboration between Uganda and DR Congo.

This is not the first time Uganda has impressed with its response to a health crisis. In the 1990s and 2000s, Uganda was widely praised for its successful campaign against HIV and Aids, which saw the prevalence of HIV drop from more than 10% to 6% in roughly a decade. The co-operation of the community is also critical in preventing infections during an outbreak, Kasujja adds. The government’s response is underpinned by a commitment to ongoing surveillance and public education, emphasizing the importance of careful precautions, and the continued collaboration between Uganda and DR Congo.

Uganda has also increased screening at the border and continues to educate the public about measures for preventing or dealing with the disease.

The situation remains fluid, with ongoing concerns about border security and the potential for the virus to re-emerge in both countries. The African continent faces a shared challenge, and the success of Uganda’s strategy provides a crucial example for other nations grappling with this complex and persistent health threat.
