The burgeoning “Make America Healthy Again” movement has dramatically reshaped the conversation surrounding nicotine, transforming it from a regulated substance to a highly publicized wellness product. Across social media, podcasts, natural medicine communities, and tech startups, prominent wellness figures and influencers are aggressively promoting nicotine as a beneficial compound, often framing it as a natural solution for enhancing focus, cognitive function, and even longevity. This shift, however, is generating significant concern amongst experts like myself, a pulmonologist-scientist specializing in nicotine addiction and its health consequences.

Initially, this rebranding stems from observations that nicotine is a naturally occurring plant compound found in tobacco and nightshade vegetables – trace amounts present in tomatoes, potatoes, and eggplant. Proponents of nicotine utilization also cite studies suggesting its impact on cognitive processes, and this has led to a broader, more expansive interpretation of its potential benefits. However, extending these findings to encompass productivity, brain health, and overall wellness goes far beyond what has been demonstrated in human studies.

The most significant health risks arise from tobacco combustion, which generates thousands of toxic substances, including carcinogens, carbon monoxide, and fine particulate matter. Repeated exposure to these combustion products can trigger cancer, chronic obstructive pulmonary disease, cardiovascular disease, and numerous other smoking-related illnesses. Nicotine’s comeback as a wellness product is largely driven by non-smoking options like nicotine pouches, or by combining nicotine with other substances marketed for cognitive enhancement – caffeine and nootropics – like supplements. While many smoking-related diseases result from the chemical products of combustion, nicotine itself has well-known negative effects on the brain and body.

Nicotine activates the sympathetic nervous system – the body’s ‘fight or flight’ response – increasing heart rate and blood pressure and disrupting sleep. Repeatedly kicking this system can place additional strain on the heart. Exposure is also of particular concern during critical phases of development. Furthermore, the extremely well-known risk that nicotine is addictive is a central concern. It activates reward pathways in the brain, acting on receptors for acetylcholine, a key neurotransmitter involved in attention, learning, and memory. Though it may temporarily increase alertness and attention, nicotine does not reproduce normal acetylcholine signaling. The effects last much longer and alter activity throughout the brain.

Within wellness communities, nicotine is increasingly presented as a cognitive enhancer that improves focus and productivity. While there are some studies indicating that nicotine may temporarily enhance attention, vigilance, reaction time, and some types of memory, the results are mixed. The research suggests that this benefit may only occur in individuals with mild cognitive impairment, and it doesn’t appear to offer any boost for healthy individuals, and may even worsen cognitive operation among higher-performing people.

Long-term users of nicotine often experience irritability, anxiety, restlessness, and impaired attention. Reintroducing nicotine often relieves these symptoms, making it difficult to determine whether any improvements reflect true cognitive enhancement or are simply the reversal of withdrawal symptoms. The relentless promotion of nicotine as a tool for stress and mood management also raises concerns. Nicotine does activate reward pathways in the brain, which can create a sense of pleasure and wellbeing – a temporary state – but this pleasure is not sustainable or beneficial for long-term health.

Research continues to explore potential benefits of nicotine, such as its effects on oxidative stress and inflammation. However, these findings have not been translated into anything that indicates nicotine promotes healthy aging, extends lifespan, or prevents age-related disease in humans. The continued proliferation of this ‘wellness’ narrative risks downplaying the serious health consequences associated with nicotine addiction and further blurring the line between genuine cognitive enhancement and potential harm.
